Academic Integrity Incident Report
Proctors and TAs should use this form to report suspected cheating, academic dishonesty, or other concerning incidents during quizzes and exams. Please submit this form as quickly as you can. You do not need to be certain that a violation occurred before submitting the form. We will consult with the course instructor and investigate. If possible, please take photos of the incident and retain any evidence (cheat sheets etc.) Remember that you can remove students from the exam room if they violate exam rules.
